Who is eligible?

• Call one of the numbers below to determine if you or your family is eligible. 

How do I apply?

• pick up or call for a mail-in application from the Medi-Cal eligibility office.  No appointment neccessary. 
• Call one of the location s listed in this brochure for information on how to apply for Medi-Cal.
• Make an appointment at one fo the sites listed to fill out an application.  Be sure and bring verification documents with you.  These include:
  • identiication
  • bank statements for month in which you are applying
  • proof of immigrations status
  • proof of income such as check stubs, unemployment, pension, etc.
  • proof of stocks bonds or land
